Billboard Japan posts some Soundscan data of CD sales by region using some Female and Male groups as examples such as ANGERME's recent single that reached weekly #1 on Billboard last week. They provide data graphs, sale ratios, look where they sell the most CDs at and where they are struggling, and see if either the female or male groups do better in that region.
ANGERME's sales are majority in the Kanto region (78.4%) and in the Kansai region (17.6%), which is 96% of their sales. Resulting in efficient promotion / advertising and sales in large cities and winning 1st on Billboard.

For better or worse, otaku culture — of which idol fandom is a subset — is an urban phenomenon. An urban area means a higher concentration of media, stores, events, and other fans. And when young people in rural areas become otaku — however that might happen — they typically move to urban areas in order to join that culture. And maybe go to school, in their free time.
Yes, I know that even the "rural" areas in Japan have large cities. It's all relative. Nagoya in the Chubu region (i.e., the red parts in that graph above) is one-sixth the size of Tokyo and one-third the size of Osaka.
